TEAMS (State Champions, Premier League Wildcards, and Wildcard Teams) will access the forms via links listed below. Please complete the Team information first, followed by the player information. THESE FORMS ARE MANDATORY FOR ALL AGE GROUPS. Region I has prepared a guide to these forms that is available by clicking on the link to the right. |
